Through NJ Family Care, the Division of Medical Assistance and Health Services (New Jersey Department of Human Services (DHS), begin the process to receive Medicaid Benefits for Adults with Disabilities

It is important to note there is either a FULL Application or a SHORT Application. BOTH formats of the application must be printed on paper and completed. Once the application is completed and signed, it may be emailed along with all required supporting documentation (scanned and attached to an email), or sent by regular mail (with copies of all documentation) to a Community Services Office (click here to view the CSO for your area).

We are all guilty of it... brb, fwiw, btw, lol... condensing words into acronyms. Sometimes they're a great way to get a point across, or describe a topic. However, there are times that acronyms point to specific terms or programs or services and in the DDD world, we are especially guilty of this!
